Agreement - This document is our General Agreement. Fill in the appropriate information as indicated on the quote we provide you and mail in with your deposit. Once we receive the Agreement and deposit we will book the date.
Questionnaires - the questionnaire is due 30 days prior to your reception. We will review at our face-to-face meeting.
Questionnaire-PDF Version - use this version if you do not have Microsoft Word. You can print the questionnaire and fill it in by hand.
Questionnaire-Word Version - use this version in you have Microsoft Word. You can fill the form in using Word, save and e-mail it back to us. (If you have trouble filling in the form let us know.)
Play List Form - this form is due along with the questionnaire.
More Song Lists - View the Song List tab for suggestions on songs for your reception as well as the categorized song lists in the file below. You can also choose to have your DJ choose the best music mix for the open dancing segment. We'll discuss your play list at our meeting.
